Is the cost of diagnostic tests like xrays, mri and scanning borne by health insurance?

If the diagnostic tests such as MRI, X-ray, ultrasound and blood tests are part of hospitalization of at least 24 hours, the health insurance policy covers the cost of these. If these are routine tests that are not followed by treatment, the insurer does not cover the costs.
